The Quality Partner position is responsible for process improvement initiatives, quality assurance and population health. The Quality Partner collaboratively identifies and assists with facilitating process improvement work with operations leaders within UnityPoint Clinic.
Organize and conduct initiatives, events and projects of varying size and complexity.
Facilitates process improvement work and quality initiatives to ensure efficient, high quality multidisciplinary care is provided.
Responsible for managing the day-to-day operational aspects of a project:
Identify resources and establish project schedules
Move projects forward to facilitate and accelerate progress
Provide front line support to improvement teams
Maintain project documentation
Provide input and assist in the development of project materials and PI tools
Provide communication and guidance to management, sponsors, and team members regarding their role in ensuring program and project success.
Monitor outcomes related to metrics and organizational goals and recognize opportunities for improvement.
Develop education and communication related to metrics.
Partner with UPC clinics, staff and providers to identify obstacles to performance improvement and work collaboratively to address these as appropriate.
Promotes incident reporting within the organization in order to identify Patient Safety issues and trends. Works with Director of Risk Management and Director of Quality and Population Health to provide Process Improvement strategies to address issues and promote a Patient Safety Culture within UnityPoint Clinic.
Collaborates with Quality and Population Health Directors and Regional Operations leaders to develop, improve, and implement a patient-centered medical home (PCMH) platform in support of population health.
Support and engage in clinical transformation work within UPC.
Build collaborative relationships that engender trust and motivate change. This includes relationships with fellow team members, operational leaders, physicians, advanced practitioners and other clinic staff.
Demonstrate initiative to improve quality and customer service by striving to exceed customer expectations.
Lead initiatives with use of data and metrics/success measures, quantifying the value of the results.
Provides monthly reports on the status of the program to the Director.
Utilize critical thinking in making independent judgment related to quality initiatives and seeking assistance when necessary.

Previous performance improvement experience.
Bachelor’s degree in health or business related field or equivalent work experience.
Ability to work evenings and weekends, when necessary.
Ability to travel within and outside of the state as required.
Previous RN or clinical experience in a medical office.
Previous clinic management experience.
Previous experience in Patient Centered Medical Home and/or Care Coordination.
Project Management certification.
